In order to make your appointment as smooth and stress-free as possible, we’ve gone ahead and provided all of the new patient information and paperwork you’ll need on the following page. Your First Visit
Your first visit is designed to be smooth, informative, and stress-free from start to finish. New patients can schedule an appointment online or by phone and complete all necessary forms securely before arriving. Insurance information may be uploaded through our patient portal or brought to your visit. If prior X-rays are available, we’re happy to transfer them; otherwise, we’ll complete a full set of digital X-rays and an intraoral scan. When appropriate, a cleaning may also be completed during the same visit. Most importantly, we take time to address your main concerns and clearly explain your oral health so you can make confident decisions for yourself and your family.
Patient Forms
To help make things easier on your end, we’ve included all of the new patient forms you’ll need to fill out on this page. We would ask that you complete them before your visit (or you can do so once you’re at our office), but if you need any help answering any of these questions, you’re more than welcome to let us know. By providing us with this information, we don’t just save time during your appointment; it also helps to inform us about how to approach your smile’s care.

Dental insurance is a wise investment to make in your and your family’s ongoing dental wellness, and at our office, we are insurance-friendly. We accept all major plans, and we’re a preferred provider for Delta Dental, Blue Cross Blue Shield, Altus, MetLife, and Dominion National. If you ever have any questions about how to make the most of your coverage, or you need assistance with filing a claim, feel free to let us know, and one of our knowledgeable team members would be more than happy to help.

We know that not everyone has dental insurance coverage, but it shouldn’t mean that you’re out of options for making your dental care more affordable.
